Upper Peninsula Health Plan Plus (HMO SNP) is a coordinated-care plan with a Medicare contract and a contract with the Michigan Medicaid program.

This plan is available to anyone who has both medical assistance from the State of Michigan and Medicare.

You can join Upper Peninsula Health Plan Plus (HMO SNP) if you are entitled to Medicare Part A, you are enrolled in Medicare Part B, and you live in the service area.

As a member of Upper Peninsula Health Plan Plus, if you are unhappy with the care or service you receive, you may file a complaint with us by going to the Grievances and Appeals page in this site's Members section. You may also file a complaint electronically by using the
Medicare Complaint Form or contacting the Office of the Medicare Ombudsman (OMO) for help by clicking here.
